Appointment Request Form

The Advisement Center offers 30 minute appointments daily, with 4pm as the last appointment time available.

Please note that completing this request form does not guarantee an appointment. A staff member will contact you in 24-48 business hours to confirm your appointment details.

All appointments will be in person. Please let us know if you have any questions.

If you need an alternative to an in person or virtual appointment, or if you need to work with an advisor outside of these hours, please email and we will work to find an accommodation for you.

In order for us to schedule an appointment with you quickly and efficiently, it is best if you list more than one day/time combination for your availability.
