Requirements for Participation in Varsity Sports

New Players (including Freshmen and Transfer Students)

The following forms must be returned to Health Service before participation in varsity sports, including sports' camps.

  1. A Connecticut State University Health Service Confidential Health Form must be completed by the student. Your health care provider must complete and sign part E. (See form for further instructions.)
  2. An Athletic Pre-Participation Evaluation for New Players must be completed and signed by your health care provider. This form must be completed in addition to the above CSU Health Service Confidential Health Form if you are participating in varsity sports.  The pre-participation physical exam must be done within 6 months of sports participation.
  3. An Authorization for Release of Information Reciprocal Disclosure must be signed.
  4. If you have any history of a heart murmur, irregular heart beat, or other cardiac problems, an echocardiogram report or a note from your cardiologist or physician clearing you for varsity sports must be enclosed.
  5. If you have had a recent injury, surgery or serious illness, or are currently under a physician’s care for any of these conditions, a note from the treating physician clearing you for varsity sports must be enclosed.

Returning Players

The following forms must be completed and returned to Health Service before the start of training.

  1. A Medical History Update for Returning Players must be completed and signed.
  2. An Authorization for Release of Information Reciprocal Disclosure Form must be signed.
  3. If you have any history of a heart murmur, irregular heart beat, or other cardiac problems, an echocardiogram report or a note from your cardiologist or physician clearing you for varsity sports must be enclosed.
  4. If you have had a recent injury, surgery or serious illness, or are currently under a physician’s care for any of these conditions, a note from the treating physician clearing you for varsity sports must be enclosed.
