Maintenance Work Order Form

The Department of Housing and Residence Life Maintenance Work Order Request Form







To assure that the above work will get done as soon as possible, I give permission for maintenance to enter my room/apartment to do the needed repairs. If requesting work for HVAC issues, I understand that the vents must be free of any items, and space in front of the unit must be cleared at least three feet away before maintenance can complete their work. I understand that maintenance staff can refuse service if the conditions of the room are not conducive to a healthy working environment. The conditions will need to be remedied by the occupants of the room prior to maintenance returning for services. I also understand that it is my responsibility to report any ongoing issues with this request, if any, directly to my Resident Director for further action. By submitting this form, I am using this as my digital signature.
For Phone Issues: Students are to call 611 from any on campus phone to report phone issues directly.

For Internet Issues: Jack #:
Housing is to submit request to Request University Computing with the jack #, student name, ID, and cell phone information.

To ensure the safety of all students and maintenance/facility staff members, work orders are to be completed when the room/apartment is not occupied by any students. Please provide three separate dates and time frames (2 hour minimum) between the hours of 7am and 3pm, Monday-Friday, in which you and any roommates will be out of your living space:

This form submission will be sent to the RD of the Residence Hall selected. Additionally, a copy of the submission will be sent to you at the WCSU email provided. Please make sure you have selected the correct building and entered the correct email before submitting.
