On-Campus Alcohol Permit<\/a> – request to be allowed to serve alcohol at any on-campus event<\/p>\n

Missed Classes Form <\/a>– to request prior permission from a professor to miss class for a particular scheduled university event (athletics competition, theater performance, etc.)<\/i><\/p>\n

Non-Disclosure Form<\/a> – to be completed if you want to make sure that otherwise non-confidential information will not be shared with anyone<\/p>\n
