    • Completed membership cards should be scanned and emailed to the SUOAF Secretary

SUOAF Performance Evaluation Form

    • This is the form your supervisor will use to evaluate your performance. If you have any questions or concerns, please contact one of your WCSU-SUOAF Officers.
