Attendance Roster

Please note: An attendance roster must be completed for any training and provided to the office of Environmental Health and Safety within 48 hours.  Along with any quiz that is provided with the training video. (The quiz only needs to be handed in if it is done on paper, not electronically)

For individuals or groups taking the yearly or new hire training series please use the respective attendance rosters. However, personnel quizzes must remain individual. NOTE: If there is no quiz for the individual or new hire but a signed attendance sheet the training will NOT be counted and the individual or new hire will have to repeat the course.
